This publication investigates changes in gut microbiome composition and clonal engraftment following treatment with fecal microbiota, live-jslm (RBL), in a phase 3 trial for recurrent Clostridioides difficile infection. Using deep metagenomic sequencing, it shows that RBL leads to rapid microbiome shifts towards a healthy profile, with persistent engraftment of beneficial bacterial strains—especially Bacteroidia—among responders. The findings establish a precise method for tracking engraftment from microbiota-based treatments and support future pharmacokinetic/pharmacodynamic studies in this field.

This study shows that Oxford Nanopore R10.4 long-read sequencing can generate near-finished bacterial genomes from both pure cultures and metagenomic samples—without the need for short-read or reference polishing. R10.4 achieves highly accurate assemblies, overcoming previous issues with homopolymer-associated errors. This allows for high-quality microbial genome recovery with simpler and more cost-effective workflows compared to hybrid approaches or those requiring extra polishing steps.
This publication shows that multiple sclerosis (MS) patients have reduced levels of the gut-derived metabolite propionic acid. Supplementing propionic acid increased regulatory T cells and reduced inflammatory Th1/Th17 cells in MS patients. Over three years, propionic acid intake was linked to fewer relapses, stabilized disability, and reduced brain atrophy, suggesting its potential as an immunomodulatory supplement in MS therapy.
